The Support at Home Program
The Support at Home program is the Australian Government’s aged care program that began on 1 November 2025, replacing Home Care Packages and the Short-Term Restorative Care program. It funds services that help older people remain safe, independent and living well at home — including clinical care such as occupational therapy.
At Healing Pathways NT, our occupational therapists support older adults across Darwin, Palmerston and the Northern Territory to maintain independence, prevent decline and keep doing the everyday activities that matter to them. We come to your home, understand your goals and environment, and provide practical, evidence-based therapy, equipment and modification recommendations that fit your life.
Eligibility for Support at Home is determined through an aged care assessment arranged via My Aged Care. We can support you whether you already have a package in place or are exploring your options — and we also work with clients who use NDIS funding or pay privately.

Who May Benefit
Occupational therapy can make a real difference at many points in later life. You or a loved one may benefit from support if you are experiencing any of the following:
Why Therapy at Home
Receiving occupational therapy in your own home offers practical and clinical advantages. Your therapist sees your environment directly — the layout, the steps, the bathroom, the furniture — identifying barriers that simply are not visible in a clinic.
That means recommendations are tailored to your real home and your real routine, not a generic plan. It removes the need to travel, lets family or carers take part, and supports better outcomes because therapy happens where your daily life takes place.
Timely occupational therapy can help reduce the risk of falls, prevent hospital admissions and support older people to keep living at home for longer.
Assessments in the home where you actually live.
Carers and family can take part in sessions.
We come to you across Darwin & Palmerston.
Therapy in context supports lasting change.
Eligibility & Funding
Support at Home is generally for people aged 65 years and over — or 50 years and over for Aboriginal and Torres Strait Islander people — who are assessed as needing support to live at home. Eligibility is determined through an aged care assessment arranged via My Aged Care.
Under the Support at Home program, clinical services such as occupational therapy are funded by the Government and do not require a participant contribution. If you have a Support at Home package, you can generally use it to access occupational therapy with HPNT, and we can work alongside your registered provider or support coordinator.
Don’t have a package yet? We can support you through NDIS funding or private arrangements, and point you toward My Aged Care if an aged care assessment may be right for you. Contact us to discuss the option that fits your situation.
